Community Needs Assessment

Description of a Community Health Needs Assessment

A community needs assessment is a thorough review and analysis of various factors within a specific community.
Our assessment of the residents in Madelia, will help us serve our patients better and encourage them to make healthy decisions that will positively impact their lives. 

Methodology

In conducting this community health needs assessment, MCHC solicited input from the community through a survey distributed
throughout the community in a variety of ways. Paper copies of the survey were mailed to every household,7,494 in total, in the
following zip codes:

  • 56041 - Hanska
  • 56055 - Lake Crystal
  • 56060 - Lewisville
  • 56062 - Madelia
  • 56081 - St. James
  • 56088 - Truman

Although zip codes outside of Madelia were included in the survey, MCHC determined that the included zip codes achieved the optimal combination of reaching the most people while managing the cost of the mailings. Between June 1, 2016 and November 30, 2018—a 2.5 year period—approximately 88% of our patients were from these six zip codes. Additionally, MCHC made the survey available on www.surveymonkey.com. Finally, paper copies of the survey were made available at Pioneer Bank, Alliance Bank, and at MCHC.
Spanish translations of the survey were also available on www.surveymonkey.com and in paper form. Community members were given one month to complete and return surveys.

We received 253 responses, including 43 from individuals who self-identified as low-income, six from individuals who identified as Latino, 130 from individuals who self-identified as elderly, and 45 from medical professionals. Finally, in this community health needs assessment, MCHC considered the results of the 2018 Watonwan County Community Health Needs Assessment.
