Why was the Foundation formed?
The Madelia Community Hospital Foundation was established in 2004 to provide charitable support for medical and educational needs of the Hospital that exceed normal operating expenses.
How will the community benefit from the Foundation?
The Madelia community benefits from a modern medical facility which offers local, up-to-date medical care.
How will a donor benefit from the Foundation?
Donors partner with the Foundation, providing the community with an up-to-date, life-saving medical facility. Donors receive full tax benefits for their contribution. The Foundation is recognized by the IRS as a 501(c)(3) charitable organization.
Donors are listed in the Hospital's Annual Report; we acknowledge donations of $1000 or more on the permanent display in the Hospital's lobby.
How will donations be utilized?
Donations help purchase specialized medical equipment and provide training for healthcare providers in our community. Foundation staff assist donors with specific giving requests.

You can mail your donation with a copy of this form to Madelia Community Hospital Foundation at 121 Drew Ave SE, Madelia, MN 56062. Foundation gift envelopes are also available at area churches, funeral homes, and at the hospital.
