Jane Malmskog, PharmD

Jane Malmskog, PharmD, works closely with patients and their provider to be sure that patients get the most benefit from their medication. Medication Therapy Management (MTM) focuses on ongoing management of the patient’s entire medication regiment with a concentration on optimizing therapeutic effectiveness, preventing adverse events, and achieving optimal medication therapy goals. Active involvement of patients is critical, empowering them to be a full participant in their own care through better understanding.

Dr. Malmskog reviews prescriptions from all prescribers that are providing care to a patient, and any over-the-counter and herbal products that the patient may be taking to identify and prevent medication issues. This may include medications not being used correctly, duplications of medications, unnecessary medications, and the need for medications for an untreated or inappropriately managed condition. She also gives in-depth medication-related education and advice to patients, family and/or caregivers to help assure proper medication use. This collaboration between the patient, prescribing provider and the pharmacist will help the patient reach the optimal goals of medication therapy.      

Dr. Malmskog may seem familiar to some community members, because she has been the Director of Pharmacy at MCHC since 2010. She has a bachelor’s degree in biology from the University of Minnesota-Morris, and her doctorate of pharmacy (PharmD) from the North Dakota State University College of Pharmacy in Fargo, N.D. She grew up in Ulen, a small town in northern Minnesota. In addition to doing medication management, Dr. Malmskog also does the daily preparation and dispensing of medications for MCHC inpatients and outpatients.
